We invite you to come to the Mid Atlantic Clinic: A Creating Wellness Center to sign up and pay your registration fee of $349.00 with cash, check or money order.   Please make checks payable to "Creating Wellness Frederick".  Click the link below to download the Registration form to complete before your visit. (You will need a PDF reader to use this link).

Remember that there are a limited number of spots available in the competition and once they are filled registration will be closed.
